Declaration of Intent and Tentative Study Plan

This form is needed to apply for the Certificate in Engineering for Energy Sustainability (CEES). Students must complete this form with a faculty member before submitting to the Energy Institute.

Final Course Plan

This form is needed to complete the CEES. Students must hand it to the supervising faculty of their capstone course for final approval.

DARS Authorization Form

This form must be used for one of the following purposes:
1. Any course that will receive credit toward fulfilling the capstone requirement. Students must complete a separate authorization form for each design project, independent study, internship, honors thesis, etc. that will count toward the capstone requirement.
2. Any student wishing to receive sustainability credits for a course that is not currently listed on the pre- approved course list.
